Oak Valley Senior League Registration Form
| The registration form should be completed prior to the sign up date. If forms are complete, it speeds up the registration process and keeps the lines flowing. Also required are 3 forms to show proof of redisency to verify that the player lives within the Oak Grove Little League boundary.
Acceptable forms of proof are, drivers license, auto registration, auto or home insurance, and utility bills. Please refer to the registration page for details on what is acceptable or not. For example, we can't accept three utility bills, but we can accept one utility bill, drivers licence and mortgage.

| These documents define the rules, that all Oak Grove Little League member must follow as well as the infractions and consequences for any behavior deemed inappropriate by the Oak Grove Little League Board of Directors.

| Sponsor information and application. Sponsorship support is vital to our financial needs. Player registration fees only cover a small portion of our leagues operating expenses.

High School Waiver Form
| CCS rules prohibit high school players from being involved with other leagues while trying out for, or are already on a High School roster. This form is required of all High School Players who wish to play with Oak Grove Little League to be waived from try outs with our league. Please contact the Senior Division Director if you have any questions.

Online Accident Report
| This form is used to report any and all accidents that occur during an Oak Grove Little League practice, as well as all league, inter-league and tournament games.
