In person Registrations are held at Round Table Pizza, 5689 Cottle Road (Santa Teresa and Cottle, next to Rite Aid). Please bring payment (cash or check), 3 proofs of residency and a COPY of player’s birth certificate.
On-line Registrations, please visit www.orll.org. Please note, that you will be
required to provide 3 proofs of residency and a copy of the player’s birth certificate prior to
tryouts. Checks and credit cards accepted with this option.
Register by Mail: send completed registration form (available at www.orll.org), 3 proofs of
residency, a copy of the player’s birth certificate and payment (check or money order) to: PO
Box 23499, San Jose, CA 95153.
